Critical Illness Claims

You may be eligible to receive a Critical Illness Benefit under your Critical Illness certificate of insurance. Please review your certificate for the eligibility requirements to receive benefits.

To submit a claim for a Critical Illness, please print the following forms below:

  1. Critical Illness Claim Form

  2. Attending Physician's Statement of Critical Illness

  3. Authorization to Release Information

  4. Consumer Privacy Notice

You will need to return the completed claim form and attending physician's statement to the insurance company at the address shown at the top of the claim form.
