Short Term Disability - Aflac
You are eligible to purchase Short Term Disability through AFLAC. If you are working while coverage in force and a covered off-the-job injury causes you to become totally disabled, AFLAC will pay the following benefits:

You can purchase a weekly benefit up to a maximum of $2,000 without providing evidence of insurability. Disability must begin within 90 days of your last treatment for the covered off-the-job injury. A full-time job is your primary job at which you work 19 or more hours per week for pay or benefits. Total disability is defined as being unable to perform the substantial and material duties of the employee's occupation; and be receiving care by a physician which is appropriate for the condition causing the disability; and not be gainfully employed or occupied in any other occupation.

B. To apply for Short-Term Disability insurance, call the EOI National Enrollment Center at 1-866-738-0386 Representatives are available Monday through Friday, 8:00 a.m. to 5:00 p.m. Mountain Time.
C. You can complete the section below and an experienced EOI Representative will contact you to evaluate your voluntary insurance needs and communicate the premiums for coverage:
