Term Life Insurance - Humana


You are eligible to purchase 20-year Term Life Insurance at competitive group rates through Humana. Employees can purchase increments of $10,000 to a maximum benefit of $300,000. You can purchase up to $150,000 without providing Evidence of Insurability (EOI). After this enrollment period, all elections for life insurance will require EOI for any amount. You are also eligible to elect 20-year Term Life Insurance for your spouse and/or dependent children. You can purchase increments of $10,000 to a maximum of $50,000 for your spouse, and increments of $5,000 to a maximum of $25,000 for your children. Your individual premium rate will depend on your age and will be paid for on a pre-tax basis.

B.   To apply for Term Life Insurance, call the EOI National Enrollment Center at 1-866-738-0386 Representatives are available Monday through Friday, 8:00 a.m. to 5:00 p.m. Mountain Time.
